Save Time and Enhance Patient Care with Session Notes

Session notes are essential for clinicians. They provide a comprehensive record of care, ensure continuity, and often serve as documentation for insurance, legal, and regulatory compliance. Until now, session notes tied to Creyos evaluations had to live outside Creyos platform, requiring clinicians to transpose assessment information manually—a process that created both risk and inefficiency.

Today, we are excited to introduce a feature that promises to make documentation of Creyos evaluations and assessment sessions fast, accurate, and easy. Say hello to Session Notes, where concise summaries of each session are automatically generated within the Creyos platform.

The Power of Automated Session Notes

Creyos now incorporates automated session notes, providing clinicians with a seamless and efficient way to document patient interactions. The benefits are twofold:

Time-Saving: Clinicians can now bid farewell to time-consuming manual data entry. With automated session notes, relevant patient information is effortlessly pulled in from reports, reducing the need for repetitive data input. This time-saving feature enables healthcare providers to allocate more time to patient care and other critical tasks.

Enhanced Accuracy: Automation minimizes the risk of errors, ensuring that session notes are consistently accurate and comprehensive. Clinicians can trust that their documentation is complete, up-to-date, and in compliance with relevant standards, improving patient safety and the quality of care provided.

How It Works

After an assessment is finished, Creyos will now automatically populate a new Session Note section with patient information from the relevant reports.

This includes essential details like assessment date, patient and administrator details, administration method, tasks, and questionnaires used – everything needed for claims and audits.

Session notes are initiated as soon as an assessment is administered. As the patient progresses, session information is automatically added, minus the final results. This information can be used to supplement information documented for reimbursement. After the session is complete, the session note is updated with the actual results from the assessment in that session.

Clinicians can access the session notes from the Patient Details screen. Simply click on the patient name and a View Note button will appear next to applicable sessions where notes were recorded. Clicking this button reveals the Session Note screen.



Fully digitized, session notes can be effortlessly copied and pasted into various platforms, such as EMR visit notes, comprehensive reports beyond Creyos Health, or referral documents – cutting out the need to manually transpose information.

Clinicians can then focus on documenting the specifics of the session, such as the patient's current condition, treatment plan, and any notes or recommendations. With this streamlined process, documentation becomes not only more efficient but also more accurate.

Currently Session Notes are available for sessions with cognitive tasks. Sessions containing questionnaires will have session notes available in the coming weeks.

By automating session notes and integrating them into Creyos, clinicians will experience greater efficiency, reduce administrative burden, and ultimately be able to deliver better patient care.
